Elle peut constituer le sympt\u00f4me inaugural dans environ 10 % des cas, exposant les patients \u00e0 une errance diagnostique prolong\u00e9e.<\/p><p>Sur le plan fonctionnel, l&rsquo;HO repr\u00e9sente une urgence \u00e0 ne pas sous-estimer : elle compromet la verticalisation, aggrave le risque de chutes et de traumatismes, et impacte significativement le pronostic vital \u2014 notamment par le risque de mort subite par d\u00e9samor\u00e7age de la pompe Le diagnostic repose sur la mesure standardis\u00e9e de la pression art\u00e9rielle apr\u00e8s 3 minutes debout (crit\u00e8re : \u0394PAs \u2265 20 mmHg ou \u0394PAd \u2265 10 mmHg) et sur le ratio \u0394FC\/\u0394PAS (&lt; 0,5 = origine neurog\u00e8ne). La s\u00e9v\u00e9rit\u00e9 se classe en HO s\u00e9v\u00e8re (syncopes au lever) et HO tr\u00e8s s\u00e9v\u00e8re (syncope d\u00e8s la verticalisation). La prise en charge associe des mesures non pharmacologiques (bas de contention, lever progressif, inclinaison du lit) et un traitement pharmacologique presseur (midodrine en premi\u00e8re ligne, fludrocortisone en adjuvant, droxidopa en ATU).
