Des prot\u00e9ines normalement solubles se d\u00e9stabilisent, adoptent une conformation anormale en feuillets \u03b2 et s&rsquo;agr\u00e8gent en fibrilles insolubles \u2014 les fibrilles amylo\u00efdes.<\/p><p>Ces fibrilles se d\u00e9posent dans les espaces extracellulaires et infiltrent progressivement les tissus, notamment le myocarde, alt\u00e9rant leur architecture et leur fonction. L&rsquo;infiltration amylo\u00efde cardiaque est responsable d&rsquo;un \u00e9paississement du myocarde, d&rsquo;une rigidit\u00e9 croissante et d&rsquo;une cardiomyopathie restrictive physiologiques.<\/p><p>La cardiotoxicit\u00e9 des fibrilles amylo\u00efdes est double : m\u00e9canique (\u00e9paississement, rigidit\u00e9, cardiomyopathie restrictive) et potentiellement directe (toxicit\u00e9 des oligom\u00e8res soluble sur les cardiomyocytes).<\/p><p>La nature biochimique du pr\u00e9curseur d\u00e9termine le type d&rsquo;amylose, les organes cibles, la pr\u00e9sentation clinique et le traitement. La dissociation des t\u00e9tram\u00e8res en monom\u00e8res instables est l&rsquo;\u00e9tape-cl\u00e9 de la fibrillogen\u00e8se TTR.<\/p><p>Dans l&rsquo;amylose ATTR sauvage (ATTRwt), cette d\u00e9stabilisation est li\u00e9e au vieillissement mol\u00e9culaire de la TTR non mut\u00e9e. Ces gammapathies se compliquent heureusement rarement d\u2019amylose mais repr\u00e9sentent plus de 60% des cardiopathies amylo\u00efdes.<\/p><h3>Principales Mutations Amylose TTR<\/h3><p>La transthyr\u00e9tine (TTR) est une prot\u00e9ine synth\u00e9tis\u00e9e par le foie sous forme de monom\u00e8re. Ces monom\u00e8res s\u2019assemblent en t\u00e9tram\u00e8res qui transportent des prot\u00e9ines (ex : hormone thyro\u00efdienne, vitamine D) dans le sang.
